Build and publish an intake form

Go to Forms and select New form. Add sections such as short text, email, choices, dates and file uploads, then choose where it lives: a public link anyone can fill in, or your clients' portals. Answers from a public form can create a new client automatically, ready for your first request.

Build the form

  1. Go to Forms and select New form.
  2. Enter a Form name and an optional Introduction.
  3. Add Form sections. For each, choose a Type: short text, long text, email, phone, number, date, single or multiple choice, file upload, or a section heading. Add Help text and tick Required where needed; for choices, list the Choices.

Choose where it lives

Under Publishing, choose Where:

  • Public link: anyone with the link can fill it in. Use Copy link to share it or put it on your website. Public forms always begin with the person’s name and email.
  • Client portals: the form appears in your clients’ portals for signed-in contacts.

Turn Accepting answers off to close the form without deleting it.

New clients from answers

For a public form, tick Create a client from new answers. When someone new submits it, Denby creates a client named after their company, if you ask for one, or the person. If the email already belongs to one of your contacts, the answers go to that client instead of creating a duplicate.

Answers appear under the form’s Answers and on the client’s page. Files uploaded in a form are stored and checked like any other file.

Intake forms are included on Studio and above.
